The forms necessary for the submission of candidacy certificates (COC) for the May 2022 elections can be downloaded from the official website of the Electoral Commission (Comelec).

Comelec declared that the various forms necessary for filing the COC are accessible at the address

“All the forms mentioned here are available in the offices of Comelec, free of charge, and can be photocopied, reproduced or downloaded from the official site of Comelec”, indicated the electoral body in its resolution n ° 10717.

Forms available online include: List of Authorized Signatories (Appendix A), Certificate of Appointment (Appendix B), Affidavit of Sec. General and chairman or chairman on the list of alternate candidates (Annex C), certificate of acceptance of appointment (Annex D), sworn authority to submit certificate of appointment and certificate of acceptance of appointment (Annex E), Certificate of Appointment and Acceptance (Annex F), Declaration of Cancellation (Annex G) and Authorization to Submit a Declaration of Cancellation (Annex G-1).

Also available online are forms for the Certificate of Nomination for President (Appendix H), Certificate of Nomination for Vice-President (Appendix H-1), Certificate of Nomination for Senator (Appendix H-2), Certificate of Nomination for Member, House of Representatives (Annex H-3), Certificate of Nomination for Provincial Governor (Annex H-4), Certificate of Nomination for Vice Provincial Governor (Annex H-5), Certificate Nomination for Member, Sangguniang Panlalawigan (Annex H-6), Certificate of Nomination for Mayor (Annex H-7), Certificate of Nomination for Vice-Mayor (Annex H-8), Certificate of Nomination for Member, Sangguniang Panlungsod ( Annex H-9) and Certificate of Membership Nomination, Sangguniang Bayan (Annex H-10).

The forms also available on the website relate to the authorization to file a certificate of candidacy (Annex I), the list of applicants in the queue area at 4:45 p.m. on October 8, 2021 (Annex J), the list of applicants in the queue area at 5:00 p.m. on October 8, 2021 (Annex J-1), Report on the non-receipt of the certificate of application (Annex K), File on the submission of the certificate of application (Annex L), File of the certificate of candidacy not received (Annex L-1), Daily report on the number of aspirants who submitted their certificate of candidacy by elective position by sex for the national and local elections of May 9, 2022 (Annex M), Withdrawal Declaration (Annex N) and Health Declaration Form (Annex O).

The deposit of COCs is set from Friday October 1 to Friday October 8.

Comelec said no one can run for public office “unless that aspirant completes and files a sworn COC in the form prescribed by the Commission for the post sought.”
